
"In August 2022, a 911 call was made from Atria Park in San Mateo. When first responders arrived, they found three residents who were suffering injuries from drinking caustic chemicals. One resident's lips had turned black. Two of them died and another was hospitalized with burns and acute respiratory distress but survived. An investigation by the California Department of Social Services found that Atria Park management normally bought 5-gallon containers of dishwashing fluid."
"Employees were putting the dishwashing liquid into unlabeled drink pitchers, which were left on the kitchen container until they were used in the dishwasher, the investigation found. On the day of the accident, Atria Park employee Alisia Rivera Mendoza allegedly poured the cleaner into a pitcher. Another worker, thinking it was cranberry juice, served the liquid in glasses to three residents. "With just a few sips they immediately went into serious distress," the San Mateo County District Attorney's Office said."
